In buildings above 11 metres, communal fire doors quarterly and flat entrance doors annually on a best-endeavours basis. Below 11 metres there is no fixed interval, but your fire risk assessment will usually set one.

What makes a fire door fail?
Most often the gap around the leaf, a damaged or painted-over intumescent seal, or a self-closing device that no longer closes the door onto the latch.

Minor items such as seals and closer adjustment, usually yes. Anything requiring a new leaf or frame is quoted separately.
What an FD actually records.
A fire door is an assembly: leaf, frame, seals, hinges, latch and closer. Change any one of them and the rating no longer holds. The inspection tests the assembly rather than the door.
The record is per door, not per building. A building-level pass tells you nothing about which door on which floor is the one that no longer closes.

Fire Safety (England) Regulations 2022
Regulation 10 requires responsible persons for multi-occupied residential buildings above 11 metres to undertake quarterly checks of fire doors in common parts and to make best endeavours to check flat entrance doors annually. In all other buildings the duty flows from the fire risk assessment under the Fire Safety Order.

Failures on escape routes are flagged as priority, because a door that does not close is the same as no door at all.
